You Don't Have to 'Just Live With It'

Perimenopause is the transition leading up to menopause, and for many women it begins in their late 30s or 40s. Hormone levels become unpredictable, often causing frustrating symptoms long before periods actually stop.

QUESTIONS & ANSWERS

Frequently Asked Questions About Perimenopause

What is perimenopause?

Perimenopause is the natural transition leading up to menopause, when your ovaries gradually begin producing less estrogen and progesterone. This stage often begins in your late 30s and into your 40s, but it can start earlier for some women. During this time, hormone levels fluctuate from month to month, which can lead to symptoms like irregular periods, hot flashes, poor sleep, mood changes, brain fog, and weight changes. Every woman’s experience is unique, and symptoms can range from mild to disruptive.

Many women are surprised to learn that the first signs of perimenopause often begin before their periods become irregular. Early symptoms may include difficulty sleeping, increased anxiety, mood changes, brain fog, fatigue, hot flashes, night sweats, decreased libido, or simply feeling “off” without an obvious explanation. While changes in your menstrual cycle—such as heavier, lighter, more frequent, or less frequent periods—are common, they often occur later in the transition. Because symptoms typically develop gradually, many women don’t recognize that hormonal changes may be the underlying cause until those symptoms begin affecting their quality of life.

Most women begin perimenopause between the ages of 40 and 45, although some notice symptoms in their 30s. The transition can last anywhere from a few years to over a decade before menopause occurs. Genetics, certain medical conditions, and lifestyle factors may influence when symptoms begin, but every woman’s timeline is different.

Usually not. Unlike many other medical conditions, perimenopause is most often diagnosed based on your symptoms, menstrual history, age, and overall health rather than hormone blood tests. Estrogen and progesterone levels fluctuate dramatically during this stage, so a single blood test may be normal one day and very different the next. Lab work may still be recommended if there’s concern for another condition that could be contributing to your symptoms.

Yes. Although fertility declines during perimenopause, pregnancy is still possible until you’ve gone 12 consecutive months without a menstrual period. If pregnancy isn’t part of your plan, contraception is still important during this transition. During your visit, we can discuss birth control options that also help manage perimenopausal symptoms when appropriate.

Hormone therapy is the most effective treatment for hot flashes, night sweats, and many other symptoms of perimenopause and menopause. For many women, it can also improve sleep, mood, brain fog, sexual function, and overall quality of life.

You don’t have to wait until your periods stop to benefit from hormone therapy. In fact, many women begin treatment during perimenopause while they’re still having menstrual cycles. Although hormone therapy has received a great deal of attention over the years, our understanding of its risks and benefits has evolved significantly. Current evidence shows that hormone therapy is safe and appropriate for most healthy women when prescribed thoughtfully and tailored to their individual needs.

During your visit, we’ll review your medical history, family history, symptoms, and treatment goals to determine whether hormone therapy is an appropriate option for you. Together, we’ll develop a personalized plan that aligns with the latest evidence and your individual health needs.

Hormone therapy isn’t the only option. Depending on your symptoms, non-hormonal prescription medications, nutrition counseling, exercise, sleep optimization, stress management, and other evidence-based treatments may provide significant relief. Your treatment plan should be personalized to your medical history, preferences, and goals—not a one-size-fits-all approach.

Yes. Many women notice that maintaining their weight becomes more challenging during perimenopause, even though their eating habits and activity level haven’t changed. As estrogen levels fluctuate, changes in metabolism, body composition, insulin sensitivity, and muscle mass can make it easier to gain weight—particularly around the abdomen—and more difficult to lose it. You may find that the strategies that worked in your 20s and 30s simply aren’t as effective anymore.

Successfully managing weight during perimenopause requires more than simply eating less and exercising more. During your visit, we’ll evaluate the factors that may be contributing to weight changes, including hormones, nutrition, sleep, stress, and metabolic health. Depending on your individual needs, your treatment plan may include nutrition assessment and counseling, continuous glucose monitor (CGM) evaluations to better understand your body’s response to food, optimization of hormone therapy when appropriate, and evidence-based medical weight loss options, including prescription medications. My goal is to help you achieve sustainable results while supporting your overall health and helping you feel your best.

Not every symptom in your 40s is caused by changing hormones. Thyroid disorders, iron deficiency, vitamin B12 deficiency, vitamin D deficiency, anxiety, depression, chronic stress, sleep disorders, certain medications, and other medical conditions can cause symptoms that closely resemble perimenopause. That’s why a thoughtful evaluation is so important. Rather than assuming hormones are the cause, I take the time to look at the whole picture and recommend testing when appropriate to ensure we’re treating the correct underlying issue.

Yes. Balanced Women’s Wellness provides secure telehealth visits for women throughout Connecticut. Virtual appointments make it easy to receive expert menopause care from the comfort of your home while still receiving individualized, evidence-based treatment. Whether you’re just beginning to notice symptoms or have been struggling for years, we’ll work together to develop a treatment plan that’s tailored to you.

If your symptoms are affecting your sleep, work, relationships, exercise, or overall quality of life, it’s worth having a conversation. You don’t have to wait until your symptoms become severe or until you’ve reached menopause. Many women are surprised by how much better they feel once they receive an accurate diagnosis and a personalized treatment plan.
